Why We Start by Assessing Skin Thickness

With lifting and skin boosters, the result turns on what depth the energy reaches and which layer the material sits in. The skin thickness that sets that depth differs between people — and between sites on the same face.

The subject he researched is the standard he works from

Dr. Jeong Kyungmuk is first author of a study in Skin Research and Technology (2023) that used ultrasound to measure how facial skin thickness varies with age, site, sex and body mass index. It documented, in numbers, that dermal thickness at the forehead, cheek and jawline differ from one another and shift across age groups.

What that means during your consultation

Ulthera Prime reaches a set depth per transducer — SMAS at 4.5mm, deep dermis at 3.0mm, superficial dermis at 1.5mm. Skin boosters likewise belong in different layers depending on the material. The same 4.5mm can pass beyond the intended layer where skin is thin, or fall short where it is thick. So before treatment we assess thickness and subcutaneous fat volume site by site, then choose the tip, the depth and the injection plane.

Ulthera Prime includes DeepSEE real-time ultrasound imaging: the transducer that delivers the energy also shows the tissue directly beneath it on screen. Before a line is delivered we confirm on that image where the dermis ends at that site, how thick the subcutaneous fat is, and how many millimetres down the SMAS actually lies — and then select the transducer. Reading facial ultrasound is the same task the published measurement study required. For us the screen is not an extra step; it is how the depth is decided.

What this does not claim

The study measured skin thickness; it did not establish the efficacy of any treatment. Accounting for thickness does not guarantee a result, and tissue response varies between individuals. What it does mean is that the depth is decided from the skin in front of us rather than a device default.
